Merchant Accounts
Application
- You may be able to apply online INSTEAD of using
the following paper form, so be sure to contact a Sales
Representative prior to completing this application. (If you have not contacted
a Sales Representative yet, please use our Quick Form.)
- PRINT all pages
of the following application.
- Complete ALL fields.
- Initial the bottom
of each page and sign both page 4 and 5 where indicated.
- Fax the completed
application to your Sales Representative along with any additional documentation.
BEFORE YOU START
The merchant application
will take approximately 10 minutes to complete. You will not be able to
save a partial application for printing later, so be sure you have all your documentation
handy. You will need the following:
- URL of your web
site (if applicable)
- Copy of Driver's
License or Positive ID.
- Voided check (Must
match business name)
- 3 months merchant
bankcard statements (if you have processed before)
- Copies of Articles of Incorporation or if partnership,
partnership agreement (if applicable.)
If you are requesting more than $10,000 in
total monthly bankcard volume, the following information is required in addition
to the above:
- 2 years past personal and business tax returns
(signed and include all pages)
- Current business financials (signed and dated)
READY TO GO!
Once you have gathered all the necessary information,
simply:
1. Submit
the setup fee (Online account only) and
2. Downloand and print the application
and agreement form (zip file)

It only takes a few minutes and you're done!
